At Dent Neurologic Institute, our goal is to provide you with the highest-quality comprehensive and compassionate patient care in Western New York.

Here you will find everything you need to schedule an appointment, understand your rights as a patient, explore our available support groups, and learn more about various illnesses and conditions, as well as the latest available treatments.

In addition, you may download patient forms to review or complete prior to your appointment. Any questions, feel free to contact us either by telephone or email, or simply ask a receptionist for assistance when you arrive.

DENT is committed to offering our patients the most comprehensive care as possible. To ensure ease of communication at your appointment we offer interpretation services should you require them.

Scheduling Appointments

To make an appointment with the Dent Neurologic Institute, call (716)250-2000, Monday through Friday between 8am and 5pm. When you call, one of our appointment consultants will ask you several questions to help better plan your visit. You will then be scheduled for the next available appointment. If this will be your first visit, you will receive a package of information needed for your visit in the mail before your appointment.

Evaluations and/or treatments may require several hours to complete, and/or additional appointments may be required to complete the initial evaluation. These subsequent visits will be scheduled at your convenience.

If you think your situation is of an urgent nature, please let us know when you call. A clinical nurse specialist will call you to discuss the changes in your condition that you are experiencing and evaluate the urgency of the situation. If necessary, your appointment will be rescheduled to an earlier date.

The Day of Your Appointment

Please arrive 15-30 minutes before your scheduled appointment to register and update or complete your medical history. Be sure to bring a photo ID, insurance card(s), prescription from your referring doctor and any co-pay, co-insurance or deductible due at time of service, and your referral form if it has not been sent previously by your physician.

In addition, please have your completed New Patient Package, as well as any recent test results, previous consultation records, and/or diagnostic reports and films from all physicians treating your condition, and a list of medications you are taking with dosing details.


Our team strives to provide support to both the patient and family in managing their health and well being. Each of our providers is here to treat you with the same professionalism and dedication they would want for their own family members. As an extension, we host a number of Support Groups at both our Tower and Orchard Park facilities.
